4. Online Appointment Scheduling — Acuity Scheduling
Our website provides access to Acuity Scheduling, a third-party online appointment booking platform. By using Acuity Scheduling through our website, you also agree to Acuity Scheduling’s terms of service and privacy policy. We are not responsible for the availability, accuracy, or terms of Acuity Scheduling’s platform.
Submitting an appointment request through Acuity Scheduling does not constitute a confirmed appointment or the establishment of a patient-provider relationship until you have received confirmation from our office. We reserve the right to decline any appointment request.
5. Telehealth Services — Zoom
When you participate in a telehealth session with us via Zoom for Healthcare, your use of the Zoom platform is also subject to Zoom’s terms of service (zoom.us/terms). You are responsible for ensuring that your device and internet connection meet the technical requirements for a telehealth session.
Participation in telehealth services is entirely voluntary. We cannot guarantee the uninterrupted availability of telehealth services due to factors outside our control, including internet connectivity issues, platform outages, or technical failures. In-person care may be necessary if telehealth is not technically feasible.
6. Social Media — Yelp
We maintain a business listing on Yelp. Your use of Yelp — including posting reviews, messages, or other content about our practice — is governed by Yelp’s terms of service, not these Terms of Use.
