How to schedule appointment for EOS at DUKE

If the provider that orders EOS is in the DUKE network:

1. If you have a Duke provider and the order for imaging has been submitted, call the Imaging Scheduling Department at 919.684.7999 to schedule an appointment for EOS imaging at Lenox Baker Children’s Hospital.

Ordering Provider is outside DUKE:

1. Have your provider’s office call 919.684.7999 to obtain a referral slip.
2. Once your provider has submitted the order for imaging, the patient should call 919.684.7999 to schedule an appointment for imaging.
3. Sign a Release of Information at Advance Physical Therapy if you would like to have a copy
   of the images and imaging report be sent to Advance Physical Therapy.

How to schedule appointment for EOS at UNC

If the provider that orders EOS is in the UNC network:

1. Confirm that UNC provider has submitted order for imaging requesting Scoliosis Study, specifying EOS Imaging.
2. Once you have confirmed order has been submitted, call the Imaging Scheduling Department at the Ambulatory Care Center at 984.974.8762, choose Option #1.

Ordering Provider is outside UNC:

1. Have your provider’s office give you a paper copy of the order for imaging. Order should specify that they are requesting EOS Imaging.
2. Patient should call Imaging Scheduling Department at 984.974.8762, choose Option #1. Scheduler will instruct patient on how to fax/email the order to UNC.
3. Once order has been received by UNC, patient can be scheduled.
4. Sign a Release of Information at Advance Physical Therapy if you would like to have a copy
   of the images and imaging report be sent to Advance Physical Therapy.
