Insurance FAQ’s
Advance Physical Therapy participates with several insurance carriers including Medicare, Medicaid (ages 21 and under), Blue Cross Blue Shield (excluding BlueValue, BlueHome and BlueLocal plans), and Tricare. We also offer a discounted self-pay rate and accept CareCredit. Not all insurance policies include benefits for Out-Of-Network providers. It is the patient’s responsibility to verify specific policy coverage provided by your insurance carrier for physical therapy, including any patient responsibility costs (ie: copay, coinsurance). When calling your insurance carrier to verify benefits/coverage for your specific policy, please specify your request is for physical therapy provided by a licensed Physical Therapist in a private, outpatient office setting. Your insurance carrier will determine coverage based on your policy.
IN-NETWORK INSURANCE CARRIERS
Advance Physical Therapy will bill your insurance carrier for services provided. Your insurance policy is a contract between you and your specific insurance carrier. Your coverage and benefits are determined by your insurance carrier. Your insurance carrier reserves the right to decide what services are medically necessary for you and which health related services they will cover. Advance Physical Therapy will verify eligibility of coverage.
OUT-OF-NETWORK INSURANCE CARRIERS
If Advance Physical Therapy is considered an out-of-network provider with your insurance carrier, as a courtesy, we will bill services to your insurance carrier upon request. However, payment in full is due at the time of service. Payment by an out-of-network insurance carrier will be paid directly to you, as the insured party.
DISCOUNTED SELF-PAY RATE
Advance Physical Therapy offers a discounted self-pay rate. Please call our office to inquire about our current discounted self-pay rates.
DO I NEED A REFERRAL FOR PHYSICAL THERAPY?
Advance Physical Therapy does not require patients have a referral. However, your insurance carrier may require that you have a referral physical therapy. Patients with Medicare, Medicaid (including Medicaid managed care plans), Tricare Prime and some BCBS policies require a referral for physical therapy. Please contact your insurance carrier to verify if your specific policy requires a referral for physical therapy.
